Chronic Care Management is a non-face-to-face service offered to Medicare beneficiaries who are suffering from two or more chronic conditions expected to last at least a year or until death. Pharmacists can provide CCM services within their scope of practice under the general supervision of a Qualified Health Plan. They are capable of performing the CCM services including collection of structured data, maintaining or informing updates for the care plan, perform management of care, provide a 24/7 access to care, document CCM services, and offer support services to facilitate CCM.
